How To Apply

1.  Register for the Online Scholarship Placement Exam

2.  Transfer students seeking current year admissions, complete 2021-2022 application.  Current 8th grade students seeking Fall 2022 admissions, complete the 2022-2023 Cleveland Central Catholic Admissions application.

2022-2023 Admissions application

3.  Submit the required documents:

  •         Birth Certificate
  •         Immunization Records
  •         Student Essay/Questionnaire
  •         Current Utility Bill (Ex: Light bill, Gas bill)
  •         Current IEP & ETR (If applicable)
  •         Legal custody document (If applicable)
  •         Two teacher evaluations (Major subject teachers/Principal)
  •         Current 8th grade students:   7 & 8th grade transcript (grades, attendance records, discipline records, state standardized test scores)
  •         Current 9th grade and transfer students:   Official transcript, attendance records, discipline records, state standardized test scores)

 

Items can be faxed to 855-692-2247,emailed to admissions@ccc-hs.org, or dropped off to Cleveland Central Catholic with our Admissions Coordinator, Ms. Yesenia Gil: Admissions, 6550 Baxter Avenue, Cleveland, Ohio 44105.  Please inform us of any questions you may have, or any assistance you may need.

 

Catholic Grade School Students

  •     Students in Catholic schools associated with the Diocese of Cleveland may apply for admission by indicating Cleveland Central Catholic as their          first choice on the application distributed by their elementary school.
  •     Follow the deadlines and procedures as determined by the Diocese of Cleveland
  •     Families residing in the City of Cleveland complete the Cleveland Scholarship Program Application
  •     Families residing outside of the City of Cleveland complete the Ed-Choice Scholarship Program Application
  •     Take a Placement Exam

 

Non-Catholic Grade School Students

  •     Submit a completed and signed application. Applications will be available in February 2021 for the following year's enrollment
  •     Submit elementary school grades (7th and 8th) and standardized test scores
  •     Have current school teacher or principal fill out an evaluation form given in the application packet
  •     Families residing in the City of Cleveland complete the Cleveland Scholarship Program Application
  •     Families residing outside of the City of Cleveland complete the Ed-Choice Scholarship Program Application
  •     Take a Placement Exam

 

Transfer Students

  •     Submit a completed and signed application. Students looking for possible enrollment for the second semester need to contact the admissions office
         to see if space is available.
  •     Submit complete high school transcripts, state standardized test scores (OST, AIR, IOWA, NWEA, MAP)
  •     Have current school teacher or principal fill out the evaluation form given in the application packet
  •     Families residing in the City of Cleveland complete the Cleveland Scholarship Program Application
  •     Families residing outside of the City of Cleveland complete the Ed-Choice Scholarship Program Application
  •     Take a Placement Exam

Please Note: If a student requires Special Education services, a copy of the Accommodation Plan (IEP, Sego, SP, 504) and the Evaluated Team Report (ETR) will also need to be submitted before your application will be reviewed. Admission to Cleveland Central Catholic High School is available to qualified applicants regardless of religion, race, gender, sexual orientation, or ethnic background.
